An Uber self-driving car struck and killed a female pedestrian in Tempe, Arizona Monday. The company immediately suspended self-driving operations in Tempe, San Francisco, Pittsburgh and Toronto.

At the time of the accident, the car was in autonomous mode with a vehicle operator behind the wheel, according to Tempe police.
Uber CEO Dara Khosrowshahi tweeted that the company is cooperating with a law enforcement investigation.

This is the first known death of a pedestrian from a driverless vehicle.
