Operation Fast and Furious: Who is Accountable?

Yesterday we informed readers that Attorney General Eric Holder was briefed at least five times in July and August of 2011 about Operation Fast and Furious, yet he claimed on May 3, 2010 during Congressional testimony he had only known about the program for "a couple of weeks."

Now, the House Oversight Committee is asking, who is accountable? And who is in charge?
