The US has the best medical system, if you can afford it. Most objective measures, such as life expectancy, do not support better quality of life in the US over other developed democracies. Truth is, the US, like Canada, provides free medical care for those who can't afford it. That's why the US government spends almost as much per capita on medicare as the Canadian government. Yet very few people, American or Canadian, know this - Bieber being one example. In his case, growing up with a single mom and not a lot of money, Canadian medicare was probably essential. As Austin Hill writes, it means those who paid more taxes subsidized his family's health care. But, as noted above, so it is in the US too.

Reports of that incident were terribly distorted. It was for medical reasons the doctors wouldn't perform the tracheotomy, and nothing to do with cost. The same decision may well have been taken by American doctors.
