John Stossel's history lesson of how Employee Benefits caused this mess will fall on deaf ears. Virtually all Americans alive today believe someone, other than themselves, should pay for their health care costs. I was around during WWII, I saw it all begin. In no time at all, young mothers were dragging their kids to doctor, for the sniffles.

Initially health insurance was strictly for catastrophic medical expenses - it functioned the way insurance should function. You pay for your own doc visits for sniffles; insurance pays when you get cancer or break your leg. It was the UNIONS who wanted richer and richer employee benefits who started the system where the health insurance pays for everything. This takes health insurance OUT of the realm of INSURANCE and just turns it into a third-party-payer system, raising costs for everyone.

it was about the same time as they started withholding taxes.

Yes. WWII. Health benefits as a job lure because there were wage controls in effect. And withholding of taxes on a "paycheck basis" because the Govt. needed the money NOW, not at the end of the year.

Blame Japan and Germany... :-)
